The forms may be completed online and then printed out, or downloaded and then filled in with a pen (please print legibly). Also, please bring the following items with you to your first appointment:

• *A copy of your DRIVER'S LICENSE
• *A copy (front/back) of your INSURANCE CARD and secondary insurance payor information if applicable.

And, it is useful to bring with you any lab. or radiology reports that may have been performed in the previous six months.
